SECTION 1 – WHAT DO WE DO WITH YOUR INFORMATION?

When you have treatment or purchase something from our practices, as part of the buying and selling process, we collect the personal information you give us such as your name, address, email address and contact telephone number.

When you browse our website, we also automatically receive your computer’s internet protocol (IP) address in order to provide us with information that helps us learn about your browser and operating system.

Email marketing: From May 25th 2018 all our Medical History Forms which you have to complete before our first appointment at our practices will carry an opt-in section for Perfect Smile Communication. With your permission, we may send you emails and or texts about our services, new products, special offers and other updates. For those who have been patients at our practices prior to May 25th 2018, you will have the opportunity to update your preferences, unsubscribe or carry on receiving email marketing. Remember, you can change your mind at anytime by notifying your local Perfect Smile practice.

 

SECTION 2 – CONSENT

How do you get my consent?

When you provide us with personal information to complete a transaction, verify your credit card, place an order, arrange for a delivery or return a purchase, we imply that you consent to our collecting it and using it for that specific reason only.

If we ask for your personal information for a secondary reason, like marketing, we will either ask you directly for your expressed consent, or provide you with an opportunity to say no.

SECTION 3 – DATA OPT-OUT POLICY: How the NHS and care services use your information

Perfect Smile is one of many organisations working in the health and care system to improve care for patients and the public. Whenever you use a health or care service, such as attending a Dental Appointment, Accident & Emergency or using Community Care services, important information about you is collected in a patient record for that service. Collecting this information helps to ensure you get the best possible care and treatment. The information collected about you when you use these services can also be used and provided to other organisations for purposes beyond your individual care, for instance to help with:

▪ Research into the development of new treatments
▪ Preventing illness and diseases
▪ Monitoring safety
▪ Planning services

This may only take place when there is a clear legal basis to use this information. All these uses help to provide better health and care for you, your family and future generations. Confidential patient information about your health and care is only used like this where allowed by law.

Most of the time, anonymised data is used for research and planning so that you cannot be identified in which case your confidential patient information isn’t needed.
You have a choice about whether you want your confidential patient information to be used in this way.

Data being used or shared for purposes beyond individual care does not include your data being shared with insurance companies or used for marketing purposes and data would only be used in this way with your specific agreement.

Our practices only use your personal health data to provide individualised care to you and do not disclose your data for any other purposes. The national data opt-out does not apply to our usage of your data and we are compliant with the policy.

 

SECTION 4 – DISCLOSURE

We may disclose your personal information if we are required by law to do so or if you violate our Terms of Service.

SECTION 6 – SECURITY

To protect your personal information, we take reasonable precautions and follow industry best practices to make sure it is not inappropriately lost, misused, accessed, disclosed, altered or destroyed.

If you provide us with your credit card information, the information is encrypted using secure socket layer technology (SSL) and stored with a AES-256 encryption. Although no method of transmission over the Internet or electronic storage is 100% secure, we follow all PCI-DSS requirements and implement additional generally accepted industry standards.

 

COOKIES

Perfect Smile use Google Analytics cookies. These are stored for 38 months on a users machine.

 

SECTION 7 – AGE OF CONSENT

By using this site, you represent you have given us your consent to allow any of your minor dependents to use this site.

We reserve the right to modify this privacy policy at any time, so please review it frequently. Changes and clarifications will take effect immediately upon their posting on the website. If we make material changes to this policy, we will notify you here that it has been updated, so that you are aware of what information we collect, how we use it, and under what circumstances, if any, we use and/or disclose it.

If our website is acquired or merged with another company, your information may be transferred to the new owners so that we may continue to market services to you.
