You have a swollen tooth, and you’re wondering whether antibiotics will clear the infection. It’s an understandable question. Dental abscesses are caused by bacteria, so it seems logical that an antibiotic should be the answer.

But antibiotics aren’t automatically needed for every dental abscess. In many cases, the priority is to deal with the source of the infection through appropriate dental treatment. Current UK guidance recommends antibiotics mainly when there is spreading infection, systemic involvement or a higher risk of complications.

Do You Need Antibiotics for Tooth Abscess?

Not always. A localised dental abscess may be managed by a dentist without antibiotics. The infection needs to be assessed and, where possible, the source dealt with, and pus drained.

Antibiotics are considered when the infection is spreading, for example with cellulitis, swelling or swollen lymph nodes, or when there are systemic signs such as a fever or feeling generally unwell. They may also be appropriate when there is a higher risk of complications. So, having an abscess does not automatically mean you need a prescription.

What About Antibiotics for Periapical Abscess?

A periapical abscess is a type of dental abscess that forms around the tip of a tooth’s root. It usually develops when bacteria infect the inside of the tooth, and the infection spreads through the root canal into the surrounding tissues.

Antibiotics for periapical abscess are not automatically needed. If the infection is localised, the priority is to treat the source of the infection and drain the abscess where appropriate. Antibiotics may be prescribed when there are signs that the infection is spreading, such as facial swelling, swollen lymph nodes or systemic symptoms including fever and feeling generally unwell.

If the infection has originated inside the tooth, antibiotics alone will not remove the source. The tooth may need root canal treatment or extraction, depending on whether it can be saved.

What Are the Best Antibiotics for Abscess Tooth?

If you’re wondering about the best antibiotics for abscess tooth, there isn’t one option that is right for everyone. If an antibiotic is clinically indicated, current UK dental prescribing guidance lists these as the first-line options.

  • Phenoxymethylpenicillin (penicillin V): The usual first choice. Its narrower spectrum makes it less likely to contribute to antimicrobial resistance. This is the main penicillin for tooth abscess option recommended in current UK dental guidance.
  • Amoxicillin: Another first-line option. It may be more suitable when taking phenoxymethylpenicillin at its recommended frequency could make following the treatment difficult. This means amoxicillin for dental abscess may be considered in appropriate circumstances.
  • Metronidazole: Appropriate for people who cannot take penicillin and may also be used alongside a penicillin in some situations involving spreading infection. Dental abscess metronidazole treatment therefore depends on the individual clinical circumstances.

Clindamycin has a restricted second-line role and should generally only be considered for severe infections when appropriate first-line treatment has not worked, following clinical review. Clindamycin also carries a higher risk of serious antibiotic-associated colitis.

Do Antibiotics Cure a Dental Abscess?

Antibiotics can help control a dental infection when they are clinically indicated, but they do not replace dental treatment. An established abscess is a localised collection of infection. Current SDCEP guidance recommends local measures to achieve drainage and remove the source where possible. Antibiotics are added when there is a clinical reason for them. This is why taking antibiotics without having the affected tooth or surrounding tissues assessed may not solve the problem.

What If Antibiotics Aren’t Working?

If you’ve been prescribed an antibiotic and you’re not improving, don’t simply assume that you need a stronger one. Your dentist may need to check whether:

  • The diagnosis is correct
  • The source of infection has been adequately treated
  • The medicine has been taken as prescribed
  • The infection is responding as expected

SDCEP recommends reviewing patients who have been prescribed antibiotics, ideally at around three days. If there has been no response after 48–72 hours, the diagnosis and effectiveness of local treatment should be reassessed before another antibiotic is considered. Do not take leftover antibiotics or someone else’s prescription. Arrange an urgent dental assessment so the infection can be properly evaluated and managed.
